    Heuro provides member-centric teams, tools, technology and personalized information to lower healthcare costs of Obesity, Prediabetes, Diabetes, Behavioral Health and more.

    Our high rates of engagement stem from our unique approach to engaging Heuro Members with an entire support team built around them. Our Heuro teammates, including Member Engagement Specialists, wellness coaches and clinicians, provide the critical support and advocacy to both Members with chronic conditions and the providers caring for those patients. The Heuro team is the glue that keeps communication and collaboration on track between Members and the many doctors the patient utilizes for care.

    The Heuro algorithms create triggers across the entire population that allow our coaches and clinicians to meet each member where they are, providing the guidance and virtual care necessary on an individual basis.

  • POPULATION HEALTH MANAGEMENT

    We believe that care belongs in the office of a member’s primary care doctor or specialist. The doctor-patient relationship is critical for the success of the health of our member, so Heuro supports the care system between this relationship.

    Heuro facilitates interpretation of provider instructions and care plans and check-in on compliance with medical care, social needs or prescriptions.

    If Heuro members do not have a current healthcare provider, we help them find a quality provider in their area and help set the appointment.

    Heuro population health programs provide members with concrete guidance to proactively manage existing conditions as well as preventative guidance for members at-risk to develop a particular condition.

    In addition to providing care to our members that have been identified with chronic conditions, our algorithms identify those that are at most risk for chronic conditions. Our team of engagement specialists and clinicians provide outreach and connect with members to get them engaged with their overall health and wellbeing. We create a customized plan for those members including everything from coaching to finding a primary care doctor best suited for them.
